Note: The terms “you” and “your” used below refer to the consenting patient or (if patient is under the age of 18) the patient’s consenting parent/legal guardian on behalf of the patient. The terms “we” and “our” used below refer to Dr Scott A. Jones and his physical therapy staff at Dunamis.

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

  1. We may disclose your health care information to other healthcare professionals within our practice for the purpose of treatment, payment or healthcare operations.
  2. We may disclose your health information to your insurance provider for the purpose of payment or health care operations.
  3. We may disclose your health information as necessary to comply with State Workers’ Compensation Laws.
  4. We may disclose your health information to notify or assist in notifying a family member or another person responsible for your care about your medical condition or in the event of an emergency or/of your death.
  5. As required by law, we may disclose your health information to public health authorities for purposes related to: preventing or controlling disease, injury or disability, reporting child abuse or neglect, reporting domestic violence, reporting to the Food and Drug Administration problems with products and reactions to medications, and reporting disease or infection exposure.
  6. We may disclose your health information in the course of any administrative or judicial proceeding.
  7. We may disclose your health information to a law enforcement official for purposes such as identifying or locating a suspect, fugitive, material witness or missing person, complying with a court order or subpoena, and other law enforcement purposes.

  9. We may disclose your health information to organizations involved in procuring, banking, or transplanting organs and tissues.
  10. We may disclose your health information to researchers conducting research that has been approved by an Institutional Review Board.
  11. It may be necessary to disclose your health information to appropriate persons in order to prevent or lessen a serious and imminent threat to the health or safety of a particular person or to the general public.
  12. We may disclose your health information for military, national security, prisoner and government benefits purposes.
  13. We may leave a message on an automated answering device or person answering the phone for the purposes of scheduling appointments.
  14. No personal health information will be disclosed during this recording or message other than the date and time of your scheduled appointment along with a request to call our office if you need to cancel or reschedule your appointment.
  15. In the event that we are sold or merged with another organization, your health information/record will become the property of the new owner.
  16. You have the right to request restrictions on certain uses and disclosures of your health information. Please be advised, however, that we are not required to agree to the restriction that you requested.
  17. You have the right to have your health information received or communicated through an alternative method or sent to an alternative location other than the usual method of communication or delivery, upon your request.
  18. You have the right to inspect and copy your health information.
  19. You have a right to request that we amend your protected health information. Please be advised, however, that we are not required to agree to amend your protected health information. If your request to amend your health information has been denied, you will be provided with an explanation of our denial reason(s) and information about how you can disagree with the denial.
  20. You have a right to receive an accounting of disclosures of your protected health information made by us.
  21. You have a right to a paper copy of this Notice of Privacy Practices at any time upon request.
  22. We reserve the right to amend this Notice of Privacy Practices at any time in the future, and will make the new provisions effective for all information that it maintains. Until such amendment is made, we are required by law to comply with this Notice.
  23. We are required by law to maintain the privacy of your health information and to provide you with notice of its legal duties and privacy practices with respect to your health information. If our Privacy Officer is not available, you may make an appointment for a personal conference in person or by telephone within 2 working days.

Dr Scott is a blessing. A rare doctor that truly cares and listens. You're never a number with Scott, his passion for... helping you get better is evident the moment you meet him. I found Dr Scott by pure luck, a late night google search after my primary care sent me to a physical therapy group that was only running me through the motions. Dr Jones was a fantastic physical therapist. His expertise and experience with "cause and effect" diagnosis helped me... within minutes of limping in for my first appointment. Back pain was eased before I left that day. He demonstrated, explained, and educated me regarding the probably issues causing my back pain--even sent some helpful video links. It's been two months since the first referral, and I can say that I am not only pain free (without any pills!) but also improving on a number of other physical performance factors needing attention. Having been to more than a dozen physical therapists since I've been in Colorado Springs (amost 20 years now), Dr. Scott Jones was by far the most effective. Highly recommended to military folk looking for some genuine expertise and results from a physical therapist.read more

I’ve been lifting weights for years but between a physically demanding job and an inability to perform full range of... motion without pain on some exercises I decided to see Dr. Scott. Dr Jones is amazing, competent beyond belief and patient. My husband is elderly, and Dr Jones worked with him until he... was stronger than he had been in years. He had gone to several physical therapists in the past and none were able to heal long standing injuries. Dr. Scott Jones desires to see his patients’ overall health improve. He helps one move forward with a better quality... life. His techniques and practice are much more than the typical ice, heat, see me every week approach. As long as one is willing to do their “homework” of exercises developed specifically for them, improvement will be seen. I know this first hand! Dr. Scott Jones is excellent. As a former Ballet Dancer having had two knee surgeries, I have received therapy by... countless PT providers in the last 16 years, and have never experienced one with the unique approach Dr. Scott uses. He projects an atmosphere of genuine concern and detailed care for each individuals physical needs. In helping me with knee complaints he shed light on and revealed other significant overlooked issues leading directly to my original pain.
